Cause of the crash edit

I've just reverted an edit to the infobox that said that the cause of the SPAD was equipment failure. You cannot blame the AWS being switched off as being the cause of the SPAD - the driver is supposed to obey the signals regardless of whether the AWS is working or not. -- Arwel Parry (talk) 19:46, 17 August 2013 (UTC)Reply

Requirement for the signaller to have been informed that the HST was in service with its AWS isolated edit

The article says that "At the time there was no requirement for the signaller to have been informed that the HST was in service with its AWS isolated". However paragraph 6.17 of the The Southall Rail Accident Inquiry Report states that "the proper procedure was for the AWS fault to be reported to a signalman". I can't find any reference for the former and so I think that the article should be updated to the latter. If I don't get any objections within a few days, I will make this change.
