How get photo reinstated and 'unclear' header removed?

As is probably obvious, I am still a newbie at Wikipedia editing.

Right now, I have two questions about this page:

First, somehow I only just noticed that last December the photograph accompanying this article was removed. The editor's explanation was, " (Removed red-linked image & caption - file deleted from Wikimedia Commons on 20 December 2013)". Can you tell me why the file was deleted from Wikimedia Commons? And, can you tell me what I must do in order to successfully reinstate it?

Second, also last December, a question mark header was added the top of the page, stating, "This article includes a list of references, but its sources remain unclear because it has insufficient inline citations. Please help to improve this article by introducing more precise citations." Since that time I have added inline citations, but the header remains. How can I get it removed?
