I believe that should be:

  • Waddell, Arizona, Buddhist temple shooting

– I'm proposing these awkwardly titled articles be renamed. WP:Copyedit#Punctuation prescribes that City, State constructs require a second (closing) comma, unless followed by other punctuation. However, using City, State as a compound modifier makes the title awkward and a little confusing to read, in running text or otherwise, whether with the comma (#3) or without it. For #4 I have added the year to add precision (there are also many Pike counties). I'm a little hesitant over #5, since Webster is the name of many things beside places. An alternative would be 2012 Webster shooting (New York), following the pattern of Harris County Sheriff's Office (Texas).
