Surrogate Consultant: Helps members navigate resistance by healthcare providers to following a valid advance directive or living will. Supplemental Advance Directive for Dementia: Downloadable form to accompany a standard advance directive in the event that a dementia diagnosis interferes with a person's right to refuse medical treatment or nourishment.
