It's very simple, you can do it yourself: Click "Upload File" on the left sidebar and then select "Old guided form" on the bottom table. If you insist to join to "keep the vote" you must renominate the Kiara Belen article over AfD and there's an easier solution if you use WP:TW for seeing a popup.
