The first thing you'll need to do is wait about six months before trying. A user with a history of abusing multiple accounts is unlikely to get an unblock request approved. Then, you'll need to log in with your original account to request the unblock. You'll need to demonstrate that you understand core Wikipedia policies and are willing to abide by them.
