This template is misleading at best. It places the text from {{UsernameBlocked}} on a user's talk page, stating that "Your user name has been blocked" when this simply is not the case. It seems to be an attempted alternative to The Way To Do It, which is to list the name on WP:AIV so an admin can block the user name if appropriate. however placing untrue statements like "You have been blocked" when this is not true is, I'd say, unhelpful.
