I have suggested a minor editing change (see below) to support the important concept of the 'essence of nursing care'. 'The CCC, capturing the essence of patient care, consists of two interrelated terminologies - the CCC of Nursing Diagnoses and Outcomes and the CCC of Nursing Interventions and Actions - classified by 21 Care Components that link the two together and enables mapping to other health-related classification systems.'

Some Supplementary Information about the CCC

I suggest adding some history of the CCC, to help understanding the formation of CCC. The concept of CCC didn't jump into the CCC we known today.

According to the http://www.ncbi.nlm.nih.gov/pmc/articles/PMC1560509/ The CCC, previously the Home Health Care Classification (HHCC), was originally created to document nursing care in home health and ambulatory care settings 6. Specifically designed for clinical information systems, the CCC facilitates nursing documentation at the point-of-care.The CCC was developed empirically through the examination of approximately 40,000 textual phrases representing nursing diagnoses/patient problems and 72,000 phrases depicting patient care services and/or actions. The use of the CCC has expanded into other settings in addition to home health care and it is claimed to be appropriate for multidisciplinary documentation.

Positive:

The article demonstrates a clear definition of the Clinical Care Classification System. The article demonstrates a contrast between it's previous editions and current which highlights the developments in design of the system. The structure of the article is good in terms that it explores the history of the system, it's major features, the model of the system, system framework and it's applications.

Suggestions:

Please fix the heading 'introduce' as this would be incorrect language, it should be changed to 'Introduction'. I would also suggest that the a gap analysis of the Clinical Care classification system also be included as every system is bound to have flaws and not including this section would suggest that it doesn't have any flaws.
