  1. Go to the Google Books page for Barker (2003) (here)
  2. Copy the URL.
  3. Paste the URL into here where it says "Google Books URL" and hit "Load"
Near the bottom of the page you will find some text beginning <ref= and ending </ref>. Copy it and replace "(Barker 2003. p215.)" below with that new text.

Your formatting in those contributions was fine, except that you don't need to include (author, date) because that information is in the reference.
The edit to Panic attack was fine and a good contribution to the article. Your contribution to Alzheimer's disease was reverted because the article already contained that information. Your Gender identity disorder contribution was deleted, because another editor didn't like it. If you want, you can go to the article's talk page and begin an argument with them about why your contribution should remain. But I've just checked their contributions history and they do a lot of editing in this subject area, so you might up against an opponent who is more knowledgeable and more committed to the subject than you. Your choice.
