This template is used at the top of a biographical article to explain that on Wikipedia, a Pakistani is referred to by their given name, unless they have a family name, which is not the same as a patronymic.

According to MOS:PATRONYMIC, if a person does not have a surname but a patronymic, then they should be referred to by the proper form of address, which is usually their given name.

Manual of Style advises to put this template at the top of an article immediately after the short description, before any deletion / protection and maintenance / dispute tags.
