The “two-thirds” is based on a widely-cited (United States) National Institute for Occupational Safety and Health (NIOSH) study, DHHS (NIOSH) Publication Number 86-110 (January 1986) (at http://www.cdc.gov/niosh/docs/86-110/) – “More than 60% of confined space fatalities occur among would-be rescuers…” and “…approximately 60% of these fatalities have involved would-be rescuers…”

The "more than 60%" was simplified to "two-thirds" in the 2000 Cal/OSHA publication “Is It Safe To Enter A Confined Space?” (at http://www.dir.ca.gov/dosh/dosh_publications/ConfSpa.pdf#zoom=100) which says “Two-thirds of all confined space fatalities occur among would-be rescuers.” on page 5.
