Sand Creek rises on the North Fork Elkhorn River divide about 1 mile (1.6 km) northeast of Willowdale School and then flows generally north to join Willow Creek about 3 miles (4.8 km) south-southwest of Holton School.[1]

Sand Creek drains 6.30 square miles (16.3 km2) of area, receives about 24.2 in/year (61 cm/year) of precipitation, and is about 2.96% forested.[3]
