There are two ways to proceed. First is to add a notice to http://www.blindallez.co.uk/History/tabid/381906/Default.aspx indicating that the text is available under GFDL. Alternatively, you may send a letter to the Wikimedia Foundation (permissions-en (at) wikimedia (dot) org) from an address associated with blindallez.co.uk , confirming that you release the text under GFDL.

I don't know how to help you. There is still no GFDL notice at [1], and there is no contact address provided at www.blindallez.co.uk. You say you have emailed confirmations several times; were the letters sent to permissions-en at wikimedia dot org, and were they sent from an address ... @blindallez.co.uk?
