Some well-intentioned but untidy editing over time has left this article muddled and lacking in structure. In the narrative, it is not always clear who did what, and why. I have tidied it up a bit and restored a reference that was apparently accidentally removed. I hope I have left it better than I found it. The whole article would benefit from a thorough review and edit by someone who is familiar with the material.
