Cleaning up refs edit

I have cleaned up the references in the History section to the best of my ability, but there is still much to be done. The section was originally written at Christianity in Burma and then moved here. The references were added using the {{ref}} and {{note}} tags, but unfortunately the author gave all the refs the same name, making it impossible to tell which refs went with which notes. In many edits, the same number of refs as notes were added, in which case I just assigned notes to refs one-to-one, but in other cases, there were more notes than refs, so I wasn't sure which note went with which ref, and had to just leave them. The diffs in question are:

  1. Four refs and six notes (oldest version of page)
  2. Three refs and four notes
  3. Seven refs and eight notes
  4. Six refs and seven notes
