On the Deep Purple album Made in Japan, at the beginning of the song "The Mule", someone (probably Ian Gillan) is heard talking to a sound technician

"Yeah, everything up here, please. And a bit more monitor if you got it. Ah?"
"...everything louder than everything else?"
"Yeah, can we have everything louder than everything else? Right."

...which may be the origin of the phrase.

It was Jon Lord, actually, who can just be heard saying it - this is then relayed by Ian Gillan. Alister, UK
