Initiation of formal designation of CMO

If you think my source is reputable (http://www.ncl.ac.uk/library/specialcollections/services/exhibitions/root_and_branch/root_and_branch.pdf) then there is not clarity that William Duncan was CMO or that John Simon became CMO in 1855. Two questions

We are told that 'The equivalent US term is Surgeon General'. Why? We are told that 'the CMO is one of six chief professional officers who advise the government'. Who are the others? 31.52.254.82 (talk) 18:46, 17 March 2019 (UTC)
