It doesn't really make much sense for an army group itself to be referred to as "Supreme Commander". dewiki mentions nothing about the group itself being called this - I presume this is a mistake borne from misreading a source. I've decided to remove it.

In short, "Army Group H" is not just the term for the army group and all units under it, but also the name for the staff, i.e. the commanding general and his immediate aides (adjutants, staff officers, etc.) that is immediately concerned with the tasks of command within the structure at the specific junction where an "army group" would be positioned (i.e. below a supreme command and above a number of field armies).
So, the staff was initially known as "Army Group H" after November 1944, but the staff was renamed on 7 April 1945 to become "Supreme Commander North West", on the background that the Allies had split the German forces in the Netherlands from the German forces in northern Germany. Both remained under the command of the same staff, but as they no longer were a coherent 'army group', the staff was renamed, so that the armies in question were now no longer under Army Group H, but they were under Supreme Commander North West instead. Think of it as the name of an office in a civilian surrounding.
