On May 19, 2019, Trainset number 18 had broken down in between the Araneta Center-Cubao and Anonas stations at 2:00 PM and was subsequently moved to the emergency pocket track in between Anonas and Katipunan stations waiting to be towed back to the depot. However at 9:15 PM, the train was reported to have moved on its own from the emergency pocket track towards the eastbound track. At this time, Trainset number 13 was going towards Anonas station station from Araneta Center-Cubao station on the same track. The runaway train was reported via radio wherein the operator was instructed to stop his train and brace for impact which injured 34 passengers. The driver of one of the two trains was reported to have jumped out of his train before the collision, sustaining wounds and bruises. Revenue operations were suspended to give way to maintenance checks, and normal operations resumed on the afternoon of the next day.[1][2]

The aftermath left Trainset 13's east facing driver cab numbered 2049 sustaining heavy damage, while the runaway Trainset 18's west facing driver cab numbered 2072 was totally wrecked from the incident. The incident resulted into further degradation of service wherein the active trainsets were effectively reduced from 7 to 5.
